Parent/Guardian Consent Form Template

Introduction

This parent consent form template is provided for gymnastics event organizers who use Gym Art Meets. The Organizer may use this form or an equivalent form that meets applicable legal requirements. This form must be signed by the parent or guardian of any gymnast under the age of 16 before the gymnast's personal information is entered into the Services.

Section A: Child Information

The following information will be collected about your child:

• Child's full name
• Date of birth
• Gender
• Competitive level
• Club affiliation
• Coach's name

Section B: Consent for Data Collection and Processing

By signing this form, I consent to the collection and processing of my child's personal information by the Event Organizer and Gym Art Inc. for the following purposes:

• Event Registration: To register my child for the gymnastics competition.
• Scoring and Rankings: To record, calculate, and publish my child's scores and rankings.
• Communication: To communicate event-related information (schedules, results, notifications).
• Safety: To ensure my child's safety during the event.

Section C: Additional Data

I also consent to the collection of the following additional data (check applicable boxes):

• ☐ Medical Information: Injuries or medical conditions relevant to my child's safety during the event. This information will be deleted within 6 months following the event.
• ☐ Profile Photo: A photo of my child for identification in the event system.
• ☐ Emergency Contact Information: My contact information in case of emergency during the event.

Section D: Floor Music

If my child participates in a floor exercise event with music:

• ☐ I consent to my child's floor music file being uploaded to the Services and associated with their profile.
• Floor music will be retained for the duration set by the Organizer and automatically deleted thereafter.
• Floor music will only be used for competition purposes and will not be shared with third parties.

Section E: Video Recording

If the Organizer has enabled the video recording feature for this event:

• ☐ I consent to my child's routines being video recorded by designated judges or recorders.
• ☐ I decline video recording of my child's routines.
• I understand that I may withdraw my consent to video recording at any time by notifying the Organizer.
• I understand that I may request the deletion of existing videos at any time.

Video Access Model

If I consent to video recording, I understand that:

• Videos will be stored securely and encrypted on Gym Art's servers.
• The Organizer and judges will have access to the videos.
• My child's videos may be accessible to me if the Organizer enables this feature.
• Videos will be automatically deleted 1 year after the event.
• Videos will never be made public without my explicit consent.
